We strongly encourage our staff to share any concerns they have in the course of their work to ensure we can provide the best care in the safest way.

Issues can be raised with a line manager, a department or service lead, a senior manager or Board member. Often, they can address the issue quickly and resolve any concerns.

Where this is not possible, or if the issue has been raised but there has not been any obvious improvement or there has been no feedback, you can contact our Freedom to Speak Up Guardian service.

The Freedom to Speak Up (FTSU) Guardian team can provide confidential advice and support to staff around concerns they have about patient safety or the way a concern has been handled.

Guardians don’t get involved in investigations or complaints, but help to facilitate the raising of concerns, ensuring organisational policies are followed correctly. 

We help to ensure that the organisation creates a safe environment and culture where speaking up is always business as usual.

How to raise a concern

Step 1: Tell someone 

If you have a concern about a risk, malpractice or wrongdoing at work raise your concern directly with your line manager, shift supervisor or mentor as soon as possible. 

You do not necessarily need evidence to speak up or raise a concern, but if you do have any evidence or information then please provide this to the person who you are speaking up to.

Step 2: Escalate your concern 

If you are unable for any reason to take step 1 or have taken it but not had a satisfactory response, then please escalate your concern to a senior manager within your department.

Step 3: Raise your concern with your Freedom to Speak Up Guardian

If you don’t feel able to raise you concern with your manager or if you are dissatisfied with the outcome of steps 1 and 2, you can contact the Freedom to Speak Up Guardian team:
