About District Nurses
District Nurses provide specialist nursing care and treatment to people who are permanently or temporarily housebound. We also visit residential care homes and run clinics in health centres.
(People with nursing needs, who can leave their home, will receive their care at their GP's surgery or local health clinic.)
We also teach and support people to maintain their own health and increase their independence through self-care. This enables them to support themselves to live well and safely in the community.
Our team includes experienced nurses and healthcare assistants with a wide range of skills and abilities.
We work with GPs and other professionals, such as hospital doctors and staff, social workers, and therapists.
The service we provide
We offer a 24-hour nursing service, 7 days a week.
We provide for planned care (not emergency care or social care).
We prioritise all our referrals as follows:
- we see urgent cases within 48 hours
- we see non-urgent and routine cases within 5 days.
A clinician determines our response time.
We are able to offer the following (as needed):
- wound care
- post-operative care
- bladder or bowel management
- management of long-term conditions
- end-of-life care.

What to expect
After you have been referred to our service, you can expect the following.
Health needs assessment
A District Nurse will carry out a health needs assessment with you and your carer. This will take place at your home.
This will help us to discuss and agree a care plan for you. It will also help us to decide how long you will need nursing care.
Let us know if you have any of the following:
- a visual impairment
- hearing loss
- other communication needs.
If needed, we can arrange an appropriate interpretation service at your assessment appointment.
Visits
We will arrange your home visits to suit your needs. We can arrange a 2-hour arrival window in exceptional circumstances only.
The time of your visits can vary from day to day. While we try to offer continuity, we cannot always guarantee this.
We will use a portable computer to keep an accurate electronic record of your care.
With your consent, we may need to share your information with your GP and other health and social care professionals.
Discharge from our service
We will discharge you from our service if:
- you no longer have a nursing need
- you can care for yourself
- your care can be given by a carer, a family member or another more appropriate service.
Preparing for our visits
We request that you follow these guidelines for every visit.
- To protect everyone from infection, our nurses need to be able to wash their hands. We appreciate your help in providing facilities for this.
- We ask that nobody smokes in your home while the nurse is carrying out your care.
- While we respect people’s customs and differences, our nurses will need to wear footwear in your home. This is for health and safety reasons.
- If you have pets (for example, dogs), we request that you keep them under control, or safely in another room, while your nurse is with you. Thank you for being considerate.
- Our nurses are here to support you at home. Please always speak to and treat your nurse with respect. Kindness helps us give you the best possible care.
