About us

The Children’s Community Nursing Service provides a range of services to babies, children and young people who have nursing needs, by delivering care in their home, clinic or community (e.g. school).

The team is made up of experienced children’s nurses with administrative support.

We work with babies, children and young people from 0-18 years of age who are registered with a Kingston or Richmond GP.

We work with children, young people, their families and the multiprofessional team to deliver high quality co-ordinated care closer to home.

Appointments

We will make every effort to book appointments in advance at a time or setting convenient for you. This may be at home, in a clinic or at school. However, for home and school visits we are unable to give a specific time.

We are not an emergency service. For any urgent medical advice, contact your GP, 111 or 999 as appropriate.

Referrals

We accept referrals for babies, children and young people up to the age of 18, registered with a Kingston or Richmond GP, with an identified nursing need.

This may include acute episodes of care, such as intravenous antibiotics, or chronic / complex health conditions, including palliative care.

We accept referrals from healthcare professionals, social care professionals, educational services, carers and families.

We will require a completed referral form, but we welcome a telephone call to discuss if you are unsure or have any questions.

If we are unable to accommodate your referral, we will signpost you to the appropriate service.

Once a referral has been accepted, a nurse will undertake a nursing assessment and agree a plan of care with the child/young person and their carers. We will make contact with the family within 2 working days.
