About us

The diabetes service in Kingston provides:

  • a consultant-led service​​​​​​
  • specialist nurse advice
  • specialist diabetes dietitian
  • telephone advice line and email service 
  • continuous glucose monitoring     
  • For Type 2 Diabetes only    

The specialist team, run clinics at Merritt Health Centre and Hollyfield House and provides specialist care for cases that are too complex to be dealt with by a patient's GP practice, but not complex enough to warrant hospital admission.

The team works with the patient to agree outcomes and then produce a care-management plan, which the patient’s GP practice can implement between visits to the specialist clinic. This approach means that patients with diabetes are receiving specialised care for their condition at the most appropriate level and do not have to make unnecessary trips to acute hospitals.

Referrals

Referrals for clinic assessments can be made by GP practices only. Speak to your GP for more information.

DESMOND Type 2 patient education

DESMOND is a nationally accredited diabetes self-management education programme for patients.

Referrals to our DESMOND programme in Kingston can be made by your GP or by self-referral using the NHS South London Diabetes Education Booking Service - Diabetes Book & Learn. You can book online or call them directly and they can assist you with your options and booking for sessions in Kingston Borough.

Diabetes retinal screening (eye) appointments

Eye Screening sessions are held at Merritt Health Centre or Surbiton Health Centre  Bookings or changes to appointments at either of these locations need to be made directly with the South West London Diabetic Eye Screening.
