Virtual wards (also known as ‘hospital at home’) allow patients to get the care they need at home, safely and conveniently, rather than being in hospital.
Our virtual ward team is made up of nurses, doctors and therapists, who can monitor your condition at home and make changes to your care and treatment without the need for you to stay in hospital.
'Step down' virtual ward
If you are a hospital inpatient and it is safe for you to do so, you may be offered the option of being looked after on the virtual ward, at home. This will mean you are still under the care of the hospital, and we will continue to check in and monitor your recovery, to make sure your condition does not get worse.

'Step up' virtual ward
Patients can also be referred to the virtual ward by a GP, or by NHS 111 or the ambulance service, if they suddenly become ill.
This helps to prevent the need for hospital admission as the patient can be monitored remotely from their own home.
