This service is for GPs and other referrers only.

This is the Single Point of Access (SPA) for healthcare professionals to use to refer adults with Richmond GPs to the following community-based services:

  • Cardiac Rehab Service
  • Community matrons
  • Community (district) nursing, including specialist services
  • Community Neuro-rehabilitation Team
  • Community Physiotherapy
  • Diabetes
  • Falls Service
  • Intermediate Care Team
  • Respiratory Care Team
  • Richmond Response and Rehabilitation Team
  • Speech and Language Therapy

The SPA team triage all referrals and ensure that they are directed to the appropriate service.

Referrals

All referrals need to be completed on the Single Point of Access referral form.
