About the Homecare Medicines Service
The Homecare Medicines Service delivers hospital prescribed medicines directly to your home or another appropriate location (for example your workplace).
The service may also include:
- delivery of other items such as dressings, needles, and syringes
- delivery of equipment such as a pump or a fridge
- nurse home visits to train you how to give yourself your own medicine
- regular visits from nurses who can give you your medicine.
Your hospital team is still fully responsible for the clinical aspects of your treatment. They will keep your GP up to date with any changes to your care.
How the service can help you
The Homecare Medicines Service makes your life easier and gives you control over your supply of hospital medicines.
It can help you in the following ways.
- You can organise delivery of your medicines to suit your own needs.
- You won’t need to wait in the hospital pharmacy queue or travel back to hospital to collect your medicines.
- Your clinical team can make automatic repeat prescriptions for you.
- Nurses can visit you at home rather than in hospital.
Delivery of your medicines
- Your homecare company will contact you directly by phone or email, or via a mobile phone app, to arrange delivery.
- A homecare van delivery driver will bring your medicines to your home.
- You can arrange to have your medicines delivered to a different address (for example, your place of work or the house of a friend or relative).
- The delivery driver will carry official identification which you can ask to see.
- You, or someone you know, will always need to sign for your medicine delivery. This means there is an official record showing that your medicines arrived safely.
To remember when you sign up for the service
- Continue to attend your routine clinic appointments, blood tests, or GP check-ups.
- Make sure the hospital and homecare service has your correct contact details (phone number, and email address if you have one).
- Make sure someone is available at the delivery place to accept deliveries.
- Make sure you are at home when a homecare nurse is booked to visit you (if you have a homecare nurse).
- Tell your homecare provider if you need to make any changes to your delivery date or nurse visit.
- Keep your medicines stored correctly. Let your hospital team know if anything is stored the wrong way.
- Check your delivery each time it arrives. Tell your homecare provider if there is anything missing or if the wrong item has been delivered.
